APPLICATIONS FOR SURROGATE CARDS


Designated borrowers may charge library materials for faculty members only when specifically authorized to do so, and only when using a special account created for that purpose.  This account is represented by a "Faculty Surrogate Card."

TA's should not be allowed to charge items to a faculty member's personal account.  Neither should the student assume responsibility on his/her own account for the faculty member's materials.

Faculty members are ultimately responsible for materials that are charged out for them by their authorized borrowers and for returning materials to the library by their due date.  Recall and overdue notices will be sent to the address specified on the application.  The card is not a "Vanderbilt  photo I.D. card"-- but just a paper library-issued card with a barcode on it. 

The card is scanned instead of being swiped.

  1. Tell the person they must get an application from the Central Card Desk, Monday - Friday 8:00 am - 4:00pm. 
  2. Remind them that the Faculty member must sign the form before returning it to Central.
