Requesting Items from the Annex

Patron will receive a YELLOW Annex request Form

The paper request form shows a URL for a Web Form on the Internet. The web form is where the request is really made, so we encourage patrons to use the URL and fill out the Web form themselves. However, if they are not comfortable with computers, they may fill out the paper form and hand it back to you. Then you will make the Web request for them.

Steps for placing the Web request for the patron:

Check the record on Acorn to confirm that the particular volume or year of the item requested is in the Annex, and that the patron's citation is correct.

Go to the Webform. (see link below after reading instructions)

Type the basic information patron and citation information as completely as you can, but do not worry if optional fields are blank.

Type "Requested by (your initials) from Divinity for the Patron (SS#)" in the comments field.

Submit the form.

Print the screen confirmation.

Place the confirmation in Sandra's IN basket.

***If it is an Annex book belonging to another library, type "PICK UP AT (insert name of library)" in the comments field. ***
